Library Cards


Those wishing to check out materials from the Powers Memorial Library must have a valid public library card. A free card is issued to any Wisconsin resident that is able to show proper identification displaying their current residential address and either their driver’s license or a state-issued identification card.

A. Adult Library Card
Any person 18 years of age or older qualifies for an adult library card who presents proper identification.  They have unrestricted access to all types of materials in the library, subject to normal regulations concerning fines; adhere to the library public code of behavior, etc.

The following documentation is acceptable upon application:

Picture identification with current address information, such as, a driver’s license or state issued identification card.

If applicant has recently moved here, applicant may present their picture identification from the previous state, along with a current phone, utility, rent receipt, lease or checkbook with current address.

B. Juvenile Library Card
Juvenile cards are issued to children who are three years of age or older.  The juvenile card will be issued upon the completion of the application form with the signature of a parent or guardian.  Juveniles are restricted from using the Internet (See Appendix A), checking out movies, and Wii games without parental approval.

The following documentation is acceptable upon application for individuals’ under the age of 18:

  1. A current school identification card from a local school.

  2. Picture identification with current address information, such as, a driver’s license or state issued identification card.

  3. A parent’s valid driver’s license or state identification card.
    (See above for adult library card criteria)

In addition to proper identification, new patrons will fill out registration forms that include the following information:

  1. Full name, including middle initial

  2. Complete street address (even if the individual receives his/her mail at a post office box)

  3. Alternate address / phone number when available

  4. Township if the individual lives outside of city limits

  5. Driver’s license or state identification

  6. Date of birth

Patron must then read and sign the Acceptance of Responsibility form.
